425 <br /> <br />SECTION l <br />INTRODUCTION <br /> <br />Section 21081.6 to the State of California Public Resources Code requires a lead or responsible <br />agency that approves or carries out a project where an environmental impact report has identified <br />significant environmental effects to adopt a Hreporting or monitoring program fbr adopted or <br />required changes to mitigate or avoid significant environmental effects." The City of Santa Aha <br />is the lead agency for the Lake Center Specific Development project and thus responsible for <br />implementation of the mitigation monitoring program. A final environmental impact report has <br />been prepared for this project which addresses the potential environmental impacts and, where <br />appropriate, recommends measures to mitigate these impacts. As such, a mitigation reporting or <br />monitoring program is required to ensure that adopted mitigation measures are implemented. <br /> <br />Section 2 describes the roles of responsible parties in implementing and monitoring the adopted <br />mitigation measures, and generally describes the program procedures. <br /> <br />Attachment A includes the list of mitigation measures, and identifies the timing of the <br />implementation or verification of each measure, the method of verificat'ion, and tile party <br />responsible for verifying the measure is complete. For each measure, the applicant is responsible <br />for the implementation of the measure, and a City representative is responsible for verily, lng that <br />the measure has been satisfactorily completed via plan check and field inspectk)n, and/or written <br />evidence submitted to the city by the applicant which verifies that the measure has been <br />satisfactorily completed. <br /> <br />Attachment B includes a sample ~compliance verification" form which can be used to report the <br />project's compliance.
