474 RESOLUTION 94-023 <br /> <br />1.0 INTRODUCTION <br /> <br />'me State of California Public Resources Code § 21081.6 (AB 3180) requires that a lead or <br />responsible agency must adopt a mitigation monitoring or reporting program when approving or <br />carrying out a project where an environmental document, either an Environmental Impact Report <br />(EIR) or Mitigated Negative Declaration has identified measures to reduce potential adverse <br />environmental impacts to levels that are less than significant. The City of Santa Ant is the lead <br />agency for the Santa Aha Gateway Center project and, therefore, is responsible for <br />implementation of the mitigation monitoring program. An EIR has been prepared for this project <br />which addresses the potential environmental impacts and, where appropriate, recommends <br />measures to mitigate these impacts. As such, a mitigation monitoring program is required to <br />ensure that adopted mitigation measures are successfully implemented. <br /> <br />Section 2 describes the roles of responsible parties in implementing and monitoring the adopted <br />mitigation measures, and generally describes the program procedures. <br /> <br />Attachment A contains the mitigation measures for the project and identifies the timing of the <br />implementation or verification of each measure, the method of verification, and the party <br />responsible for verifying that the measure is successfully implemented. The applicant is <br />responsible for the implementation of each measure, and the City representative is responsible for <br />verifying that the measure has been satisfactorily completeti tiuough plan check, field <br />inspections, and/or written evidence submitted to the City by the applicant, or the applicant's <br />agent, which verifies that the measure has been successfully implemented. <br /> <br />Attachment B contains a sample "compliance verification" form which can be used to report the <br />project's compliance of each measure.
