Cal;fom;a 90503-1643 <br />(310) 370.8370 <br />(310) 370.2474 FAX <br />www.hyg;enetech.com <br /> <br />City of Santa Ana Risk Management Division <br />Post Office Box 1988 M-28 <br />Santa Ana, California 92702 <br /> <br />Proposal No.1 0207045P <br /> <br />Attention: <br /> <br />Emilyn Buenafe <br /> <br />Regarding: <br /> <br />Proposal for a Limited Indoor Air Quality Investigation <br />City of Santa Ana - Animal Care Shelter <br /> <br />Dear Ms. Buenafe: <br /> <br />Hygiene Technologies International, Inc. is pleased to present the following proposal to perform a limited <br />indoor air quality evaluation survey at the City of Santa Ana Anirnal Care Shelter facility referenced above. <br />Based on the information that you provided, we developed a sampling strategy designed to: 1) determine <br />airborne concentrations of viable and nonviable fungi (yeasts and molds), fibrous dust, total dust, volatile <br />organic compounds (VOCs), nitrogen dioxide (NO2), formaldehyde, carbon monoxide (CO), and carbon <br />dioxide (CO2); 2) record air temperature and relative humidity data; and 3) evaluate the moisture content <br />in selected building materials. <br /> <br />If appropriate, we would plan to interview selected individuals in order to gather information that may be <br />useful in our investigation. The survey generally requires approximately four to six hours of time. FoIlowing <br />the survey, we would plan to have all non-biological samples requiring analysis submitted to a laboratory <br />accredited by the American Industrial Hygiene Association (AIHA). All biological analyses would be <br />performed by aerobiologists certified by the American Acaderny of AIIergy and Immunology. FoIlowing <br />receipt of all analytical data, we would plan to prepare a comprehensive report containing background <br />information, observations, exposure results, supporting information, applicable exposure limits, conclusions, <br />and, if any, recommendations. All professional services would be performed by or under the direction of <br />industrial hygienists who are certified in comprehensive practice by the American Board of Industrial <br />Hygiene and who are registered Professional Engineers with the State of California. <br /> <br />We anticipate that the fees to perform the services outlined will not exceed $4,000.00, with the following cost <br />breakdown: <br /> <br />Survey preparation and field time by industrial hygienist <br />(9.0 hours at $125.00 per hour) <br />Report preparation (4.0 hours at $175,00 per hour) <br />Project management (1.5 hours at $350.00 per hour) <br />Word processing and clerical work (2.0 hours at $40.00 per hour) <br /> <br />$ 1,125.00 <br />$ 700.00 <br />$ 525.00 <br />$ 80.00 <br /> <br />Los Angelos.
