Description of W ark - in space below, describe the program to be funded during [he 12-month contract <br />period including Sen-ices to be provided, progmm goals, client characteristics, & how grant funds will be utilized. <br />Anaheim Interlaith Shelter provides transitional housing for homeless families with dependent <br />children. (htr program provides 9 months in a furnished, private apartment in addition to <br />extensive supportive services including but not limited to weekly case management, funding for <br />free childcaze, transportation assistance, counseling, life skills workshops, apartment deposit and <br />f rst month's rent. <br />Our program focuses on one main goal - to transifion our families from homelessness and <br />instability to pernianent housing and self-sufficiency. Every service we provide, including those <br />listed above, are the steps we take toward that goal. <br />Santa Ana ESG funding wilt provide for 3 key components: case managemen~ salaries which is <br />where the work hegms.
