EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 300 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Youth will participate in a Sumner Youth Outreach Program. Programs will include an "Eazly <br />Birds" program that will provide a summer day camp experience, swimming lessons, daily free <br />lunch, a teen volunteer program, computer learning activities, and life skills program. <br />Program goals include - <br />• 100 different youth will participate in the Early Bird program <br />• 120 youth will increase swimming ability by one level. <br />• 70 teens will successfully complete the Teen Volunteer Program <br />• 50 youth will participate in a life skills program. <br />• 50 youth will participate in computer leazning activities. <br />• 75 youth will receive a free lunch daily. <br />Youth participants will be between the ages of 6-18, will be residents of Santa Ana, and will <br />come from moderate to low-income families. <br />Grant funds will be utilized for staff salazies, program supplies, office supplies, and swim lesson <br />fees. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 -September 30 <br />Quarter 2: October 1 -December 31 <br />Quarter 3: January 1 - Mazch 31 <br />Quarter 4: April 1 -June 30 <br />300 Persons <br />Persons <br />Persons <br />Persons <br />Total 300 Persons <br />Invoicing Schedule <br />Estimated the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 -September 30 <br />Quarter 2: October 1 -December 31 <br />Quarter 3: January 1 -March 31 <br />Quarter 4: April 1 -June 30 <br />$ 38.000 <br />Total $ 38.000 <br />
