EXHIBIT A Scope of Work Annual Accomplishment Goal I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- month contract period = 300 PERSONS II. Description of Work - in space below, describe the program to be funded during the 12-month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. Youth will participate in a Sumner Youth Outreach Program. Programs will include an "Eazly Birds" program that will provide a summer day camp experience, swimming lessons, daily free lunch, a teen volunteer program, computer learning activities, and life skills program. Program goals include - • 100 different youth will participate in the Early Bird program • 120 youth will increase swimming ability by one level. • 70 teens will successfully complete the Teen Volunteer Program • 50 youth will participate in a life skills program. • 50 youth will participate in computer leazning activities. • 75 youth will receive a free lunch daily. Youth participants will be between the ages of 6-18, will be residents of Santa Ana, and will come from moderate to low-income families. Grant funds will be utilized for staff salazies, program supplies, office supplies, and swim lesson fees.
