Number of unduplicatcd Santa Ana residents to be served with grant funds during the 12- <br />nonth contract period = 1,600 PERSONS <br />I Description of Work - in space below, describe the program to be funded during the 12-month contract <br />3enod including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />The SOS Free 'Medical and Dental Clinic provides high quality comprehensive medical <br />and dental care to medically indigent residents of Santa Ana and all of Orange County. <br />Services include diagnosis, treatment, lab work, medications, education and <br />comprehensive access to medical and dental specialists. The goal is to serve those who <br />cannot receive care at another facility due to lack of funds, insurance and ineligibility for <br />government programs. Patients are predominantly Latino living at or below 200% of the <br />federal poverty guidelines. Many can be characterized as the "working poor" - their <br />income too low to provide housing, medical care, food and other basic living essentials <br />for their families in expensive Orange County.
