Number of unduplicatcd Santa Ana residents to be served with grant funds during the 12- nonth contract period = 1,600 PERSONS I Description of Work - in space below, describe the program to be funded during the 12-month contract 3enod including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. The SOS Free 'Medical and Dental Clinic provides high quality comprehensive medical and dental care to medically indigent residents of Santa Ana and all of Orange County. Services include diagnosis, treatment, lab work, medications, education and comprehensive access to medical and dental specialists. The goal is to serve those who cannot receive care at another facility due to lack of funds, insurance and ineligibility for government programs. Patients are predominantly Latino living at or below 200% of the federal poverty guidelines. Many can be characterized as the "working poor" - their income too low to provide housing, medical care, food and other basic living essentials for their families in expensive Orange County. Grant funds will be utilized to partially cover the salaries for a nurse practitioner and the dental director who provide direct medical and dental care to Santa Ana patients.
