EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 50 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Goal: To help low-income people access quality health services and navigate the delivery systems as well as <br />empowering consumers to be active participants in their own healthcare and in planing programs designed to serve <br />them. LASOC is requesting funding to assist with direct services. The following objectives relate to the direct <br />service component of HCAC. <br />Obiective(s): <br />1. Help 50 low-income residents of Santa Ana resolve problems related to access to healthcare services or <br />programs. <br />Task/Action: <br />(A) Screen applicant for eligibility through the hotline Monday through Friday and provide advice and counsel to <br />eligible clients. <br />(13) Represent clients who need in-depth services. <br />2. Help low-income Santa Ana Residents become more informed and educated about their rights under current <br />laws and regulations, particularly related to managed care. <br />Task/Action: <br />(A) Provide information to eligible clients regarding their specific health care provider and insurance program. <br />3.
