EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 275 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />The Mental Health Association of Orange County provides Essential Services for the City of Santa Ana Outreach <br />Program. S taff e mployed through t he r equested funds will provide a wide r ange o f services to the mentally i 11 <br />homeless in the City of Santa Ana, including but not limited to outreach, engagement, assessment and referral <br />services, case management, counseling, assistance in accessing emergency, transitional and permanent housing, <br />assistance in obtaining federal, state and local assistance in accessing medical and mental health treatment, <br />transportation and employment services. <br />Since its creation in 1995, The City of Santa Ana Outreach Program has helped hundreds of mentally ill homeless <br />adults in Santa Ana to overcome barriers to service. This has been accomplished through the stationing of an <br />Outreach Worker at existing homeless service providers in Santa Ana, adding mental health expertise and additional <br />resource knowledge to the providers' existing level of services. <br />Goals and Objectives for the fiscal year 2003-2004 <br />GOAL: Increase access to services for the homeless mentally ill that will facilitate their transition into <br />stable housing and appropriate treatment programs. <br />OBJECTIVE(S) <br />1. Make contact with five homeless Santa Ana residents, (unduplicated), each week and <br />preliminarily screen for mental illness and assess for service needs. <br />2.
