EXHIBIT A Scope of Work Annual Accomplishment Goal I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- month contract period: �330_ PERSONS II. Description of Work - in space below, describe the program to be funded during the 12-month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. This program specifically is designed to help persons who are unable to financially continue paying their rent temporarialy. The goal of preventing homeless would be effectively carried out by recruiting and training volunteers to do extensive assessment an intake on clients as the come and telephone in for assistance. Our goal is to assist 47 families, and 138 homeless individuals. We will communicate with Community Action Partnership in the Clearing to avoid duplication and misuse of funds. This service is scheduled to be done on Tuesday and Thursday so that the confidentiality is protected of the client all other days the office is frequented by many people. We will also require the client to participate in Life Skills Prgrams.The funds will used to provide direct services to the clients.
