Intake/Assessment <br /> <br />After the Outreach Worker has identified potential participants, eligibility will be <br />established. After eligibility is determined, staff will create a file for each <br />participant. The file will hold up-to-date and relevant participant information such <br />as the program enrollment form, TABE testing results (to determine Basic Skill <br />levels), eligibility documentation, and a copy of the students completed Career <br />Prep work (application, r~sum~, goal setting forms). In addition, pre-and post- <br />assessments (determining pre-employment skills) will be in all participant files. <br />The pre-employment skills test will help determine the youths need for job <br />readiness training. Program managers will inform santa Aha W/O/R/K Center <br />staff as to the assessment results for each participant. <br /> <br />Case Management <br /> <br />The Santa Ana W/O/PJK Center will have the responsibility of assigning youth to <br />a Case Manager. Career Prep Program Managers will provide appointment <br />reminders to participants on a needed basis. Career Prep Program Managers <br />will be available to assist case managers in determining appropriate program <br />goals for the participants and will conduct weekly phone contact with each <br />participant. All information acquired over the phone will be shared with the <br />participant's Case Manager. In addition, W/O/PJK Center Case Manager's will <br />be invited to meet with program participants at the Boys & Girls Club facility. <br />Case Managers will also be invited to attend participant graduation celebrations <br />and other appropriate program related events. <br /> <br />t! <br /> <br /> <br />
