EXHIBIT B <br /> <br />STATEMENT OF WORK <br /> <br />Scope of Work. City hereby engages contractor as an independent contractor to <br />perform the following described work, and contractor hereby agrees to perform <br />said work upon the terms and conditions here-in-after set forth. <br /> <br />A. Specifically, contractor shall perform the following services: <br /> <br />Contractor shall assess each participant using a standardized <br />assessment tool within thirty (30) days of enrollment. If <br />assessment supports a Basic Skills deficiency, reading and math <br />must be the first service to be delivered to participant. <br /> <br />Contractor shall identify through prescribed assessment which of <br />the ten elements as noted below will be prescribed for each <br />participant. Chosen elements will be noted in each participant's <br />Individual Service Strategy (ISS). Services needed but not <br />provided by Contractor must be coordinated with the Service <br />Navigator and other youth partners. <br /> <br />The ten elements are: <br /> <br />Tutoring, study skills and instruction leading to completion of <br />secondary school, including dropout prevention strategies. <br />Alternative secondary school services, as appropriate. <br />Summer employment opportunities directly linked to <br />academic and occupational learning. <br />Paid and unpaid work experiences including OJT, job <br />shadowing and internships as appropriate. <br />Occupational skills training as appropriate. <br />Leadership development opportunities, which may include <br />community service and peer-centered activities encouraging <br />responsibility and other positive social behaviors during non- <br />school hours as appropriate. <br />Supportive services <br />Adult mentoring for the period of participation and <br />subsequent period, for a total of not less than 12 months. <br />Follow-up services for not less than 12 months after the <br />completion of participation. <br />Comprehensive guidance and counseling, which may <br />include pregnancy prevention, drug and alcohol abuse, <br />counseling and referrals, as appropriate. <br /> <br /> <br />
