EXHIBIT A Scope of Work Annual Accomplishment Goal I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- month contract period = 80 PERSONS II. Description of Work - in space below, describe the program to be funded during the 12-month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. Description of program to be funded: Human Options Outreach program in Santa Ana provides counseling services to Santa Ana residents. This Outreach program is under the umbrella of Human Options Community Resource Center (CRC), which is a Outreach Center serving the Southwestern portion of Orange County. Santa Ana Outreach program receives support from the CRC in staffing and administration. Human Options Outreach program in Santa Ana is located at the Minnie Street Family Resource Center and Corbin Family and Community Center. Through this Outreach program we currently provide services to victims of domestic violence, including Individual, Family, Children's Counseling, educational groups to adult women, a Children's group, and Community Education to children and youth at Santa Ana schools. Services to be provided: (1) Provide (1) Personal Empowerment Program group (PEP), which is a group focused on providing education and support to victims of domestic violence who are referred by our local courts, police department, and community service agencies (2) Provide individual child, adult and family counseling to victims of domestic violence Program goals: The goal of our program is to help battered women, their children and our community break the cycle of domestic violence. Client characteristics: The majority of the population we serve are Spanish-speaking, low-income disadvantaged women and children, and many are immigrants. They are all victims of domestic violence and reside in Santa Ana. The average age of the clients in the program are 25-34 years of age and are Latino. Grant funds: The grant funds in the amount of $17,000 we have been awarded will be used to fund a Counselor III, which is a master -level Social Worker with at least three years of counseling experience who is bilingual in English/Spanish,. for .44 FTE Schedule of Performance Estimate the number of unduplicated Santa Ana residents to be served during the 12-month contract period per quarter: Quarter 1: July 1 — September 30 Quarter 2: October 1 — December 31 Quarter 3: January 1 — March 31 Quarter 4: April I — June 30 10 Persons 20 Persons 30 Persons 20 Persons Total 80 Persons Exhibit A
