EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 80 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Description of program to be funded: <br />Human Options Outreach program in Santa Ana provides counseling services to Santa Ana residents. This Outreach <br />program is under the umbrella of Human Options Community Resource Center (CRC), which is a Outreach Center <br />serving the Southwestern portion of Orange County. Santa Ana Outreach program receives support from the CRC <br />in staffing and administration. Human Options Outreach program in Santa Ana is located at the Minnie Street <br />Family Resource Center and Corbin Family and Community Center. Through this Outreach program we currently <br />provide services to victims of domestic violence, including Individual, Family, Children's Counseling, educational <br />groups to adult women, a Children's group, and Community Education to children and youth at Santa Ana schools. <br />Services to be provided: <br />(1) Provide (1) Personal Empowerment Program group (PEP), which is a group focused on providing <br />education and support to victims of domestic violence who are referred by our local courts, police <br />department, and community service agencies <br />(2) Provide individual child, adult and family counseling to victims of domestic violence <br />Program goals: <br />The goal of our program is to help battered women, their children and our community break the cycle of domestic <br />violence. <br />Client characteristics: <br />The majority of the population we serve are Spanish-speaking, low-income disadvantaged women and <br />children, and many are immigrants. They are all victims of domestic violence and reside in Santa Ana. <br />The average age of the clients in the program are 25-34 years of age and are Latino. <br />Grant funds: <br />The grant funds in the amount of $17,000 we have been awarded will be used to fund a Counselor III, which is a <br />master -level Social Worker with at least three years of counseling experience who is bilingual in English/Spanish,. <br />for .44 FTE <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April I — June 30 <br />10 <br />Persons <br />20 <br />Persons <br />30 <br />Persons <br />20 <br />Persons <br />Total 80 <br />Persons <br />Exhibit A <br />
