EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 225 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />The Lighthouse Learning Center offers academic, social, and spiritual programs to children, teens, adults, and the <br />community at Minnie Street in Santa Ana. Our goals are to increase academic success, self-sufficiency and <br />responsibility, tolerance for diversity, overall preparedness for successful adulthood and leadership development. <br />Additional focuses are on the arts and technology. We serve families living in the poverty-stricken community <br />known as Minnie Street representing the most economically disadvantaged populations in Orange County. <br />Grant funds will be utilized to equip these children and adults with essential life and vocational skills so that they <br />can succeed in their adult life achieve their highest potential, and ultimately give back to the Santa Ana community. <br />For example, CDBG funds will provide academic curriculum for elementary -aged children; leadership programs for <br />children and teens; and college preparation curriculum, groups, and trips for teens. A part-time computer lab <br />technician will also be provided in order to support our computer lab on a daily basis. Funds will also provide <br />costumes and supplies to the children's dance and art programs. Finally, funds will provide childcare for our on- <br />going and rapidly growing adult education classes. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />225 Persons <br />Persons <br />Persons <br />Persons <br />Total 225 Persons <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter l: July 1 — September 30 $ 2,000 <br />Quarter 2: October 1 — December 31 $ 2,000 <br />Quarter 3: January 1 — March 31 $ 4,000 <br />Quarter 4: April 1 — June 30 $ 7,000 <br />Total $ 15,000 <br />Exhibit A <br />
