EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 40 PERSONS <br />IL Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />The On Track Teens Transforming Lives through Mentoring Program matches at -risk 3`a, 0 and 56' grade <br />students with qualified high school students who are trained as mentors. Through a behavioral life skills training <br />process, they are provided with: comprehensive training in conflict resolution skills, positive youth development <br />activities, and recognition for making good choices. Key teen mentors are given the opportunity to become <br />"Ambassadors of Peace." The program will match 40 Santa Ana youth: 20 high school students ("mentors") with <br />20 at -risk Kennedy Elementary School 3`d, 4`s and 5`s grade students ("mentees"), and train them in conflict <br />resolution and asset -building, while providing youth -to -youth mentoring and recreation in an after -school <br />environment. <br />The 40+ mentors and mentees will be provided with 25 hours of certified Conflict Resolution Training, as well <br />as weekly Youth Asset Building sessions. The training/mentoring sessions will be conducted on a weekly basis <br />throughout the school year. Key teaching strategies of the training include hands-on activities, role-playing, group <br />brainstorming, and cooperative activities. Each class will last for 3 1/2 hours, during which time students will also <br />be fed nutritious snacks, and will have at least 30 minutes of "fun time" together (i.e. basketball, handball, games, <br />etc.). Since academic achievement is also a priority, the mentees have one hour of tutored "homework time" <br />during each session, before the mentors arrive. The City of Santa Ana CDBG funds will be used to support direct <br />program expenses only, including program staff salaries, supplies, special events, and local mileage.
