EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 2,415 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Saint Joseph Ballet provides comprehensive, integrated programs —dance training, performance <br />opportunities, academic services, college scholarships, family services, mentoring and enrichment, and <br />community outreach —that support the educational, social and personal growth of low-income youth and <br />families. <br />Our mission is to empower low-income youth from diverse backgrounds to strengthen self-esteem, self- <br />discipline and a sense of accomplishment through dance, academic and family programs. We intend to <br />serve at least 280 Santa Ana youth in our Year -Round Programs who qualify under the "low/moderate" <br />income criterion and will participate on scholarship, and 2,000 Santa Ana students through our outreach <br />program, DanceFree Weeks. Further, Saint Joseph Ballet has a goal to strengthen and deepen the impact <br />of our services on youth and families. Objectives include securing funding for the creation of an onsite <br />performance space, academic tutoring for at least 150 Santa Ana students, free family seminars attended <br />by at least 115 parents, and assistance for Santa Ana families in need of crisis intervention and/or referrals <br />to other community resources. <br />A typical student in our Year -Round Programs is one who is from a family of five living on an average <br />income of $2,296/month. 96% qualify as low-income and participate free of charge. <br />Funds will be utilized to cover a portion of the salaries of our Student Coordinator and Academic <br />Advisor, program staff members involved in. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />265 <br />Persons <br />2,075 <br />Persons <br />75 <br />Persons <br />0 <br />Persons <br />Total 2,415 Persons <br />Estimated the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />$ 16,625 <br />$ 16,625 <br />$ 11,750 <br />$0 <br />Total $ 45,000 <br />Exhibit A <br />
