Figure 15: Administrative Tasks Associated with the Parenting Skills Training. <br />CHCADA will: <br />· Case manage the services provided to the youth and coordinate them with those provided to <br /> the parent; <br />· Assess overall needs of each family after the first parenting workshop and ensure that the <br /> parents receive case management support and referrals; <br />· Maintain documentation of parental participation in not only the parenting class but in case <br /> management as well; <br />· Provide to the Network a written assessment not only of the parent's progress in improving <br /> parental skills, but of the overall familial network as a social support to the youth with <br /> recommendations for remediation strategies; <br />· Provide a "Graduation" ceremony for parents completing the parenting training at which time <br /> parents will receive a copy of the Certificate of Completion; <br />· Utilize the "graduation" as an opportunity to formally and ceremonially prepare parer~ts to <br /> support their youth; and <br />· Schedule workshops, work with the Network, counselors, mentors, etc. in preparing parents <br /> to support youth. <br /> <br />Task 9: Provide Mentoring and Other Services <br /> Most youth will complete the program and reenter the precise social-economic <br /> <br />environment that initially served as an incubator for their dysfunctional behavior. Thus, <br />initiatives that create cognitive change are needed so that the youth, confronted with <br />choice, will alter the direction of their decision-making and their behavior. The <br />mentorship program will be designed to achieve such an outcome. However, the youth <br />and their families will need support in negotiating relationships with the labor market and <br />other institutions so that needed services can be accessed. Last, the youth will be <br />monitored and guided along a new behavioral trajectory. In order to facilitate these <br />outcomes, the services in the subsections below will be provided. <br /> <br />9.1. Provide Services as Developed in Each Youth's Case Plan <br /> CHCADA will complete a comprehensive assessment of the youth's mental, <br /> <br />emotional, social, health, academic, and other needs. Utilizing structured assessment <br />tools, interviews with the youth's family and schools, observational data gathered by the <br />counselors, and self-reported information, CHCADA's staff will provide a Case Plan. <br />This plan will serve as a roadmap to the provision of customized services to the youth. <br />CHCADA's Project Director, Project Manager, the youth's assigned Counselor, the <br />youth's Mentor, the Counselor/Case Manager, and the Counselor/Job Developer will <br /> <br />CHCADA's Response to RFP# PY2002 Page 30 <br /> <br /> <br />
