. . EXHIBIT B STATEMENT OF WORK Scope of Work. City hereby engages contractor as an independent contractor to perform the following described work, and contractor hereby agrees to perform said work upon the terms and conditions here-In-after set forth. A. Specifically, contractor shall perform the following services: 1. 2. j. Contractor shall assess each participant using a standardized assessment tool within thirty (30) days of enrollment. If assessment supports a Basic Skills deficiency, reading and math must be the first service to be delivered to participant. Contractor shall identify through prescribed assessment which of the ten elements as noted below will be prescribed for each participant. Chosen elements will be noted in each participant's Individual SerVice Strategy (ISS). Services needed but not provided by Contractor must be coordinated with the Service Navigator and other youth partners. The ten elements are: a. Tutoring, study skills and instruction leading to completion of secondary school, including dropout prevention strategies. Alternative secondary school services, as appropriate. Summer employment opportunities directly linked to academic and occupational learning. Paid and unpaid work èxperiences including OJT, job shadowing and internships as appropriate. Occupational skills training as appropriate. Leadership development opportunities, which may include community service and peer-centered activities encouraging responsibility and other positive social behaviors during non- school hours as appropriate. Supportive services . Adult mentoring for the period of participation and subsequent period, for a total of not less than 12 months. Follow-up services for not less than 12 months after the completion of participation. Comprehensive guidance and counseling, which may include pregnancy prevention, drug and alcohol abuse, counseling and referrals, as appropriate. b. c. d. e. f. g. h. i.
