<br />. <br /> <br />tit <br /> <br />OCCC will also accept referrals of potential ems from the Santa Ana WORK Center <br />navigator, TSJ, and other youth service providers. OCCC will conduct an initial <br />assessment to determine the potential ems' suitability for participation in the OCCC <br />program. The OCCC WIA Coordinator will work with the WORK Center to ensure all <br />information is collected and processed in a timely manner to ensure eligibility and <br />prompt delivery of services. <br /> <br />Intake 1 Assessment <br />Once determined suitable for participation in OCCC's comprehensive youth <br />development program, the OCCC WIA' Coordinator will conduct an initial intake and <br />assessment: <br />. To ascertain the participant's supportive service needs; <br />. To determine academic needs; <br />. To develop an appropriate educational development plan (e.g., enrollment in the <br />Charter School for high school completion or ROP; GED preparation schedule, <br />etc.); <br />. To develop an individual service plan; <br />. To select a suitable occupational training component (Santa Ana Parks <br />Maintenance or Construction); and <br />. To be oriented to the OCCC experience. <br /> <br />Case Management . <br />In addition to conducting the ems' initial intake and assessment, the OCCC WIA <br />Coordinator will be responsible for weekly monitoring of each Santa Ana ems' training <br />and work performance. They will also meet with the Corpsmember on a weekly basis. <br />Case management activities will be conducted more frequently as needed on a case- <br />by-case basis. <br /> <br />The WIA Coordinator will quickly resolve issues as they arise, make and/or facilitate <br />appropriate supportive service referrals, and follow-up on referrals to facilitate <br />successful program participation and completion. The OCCC WIA Coordinator will <br />advocate on behalf of Santa Ana participants and also act as a liaison between OCCC <br />and the Santa Ana WORK Center navigator. <br /> <br />Program Services 1 Parent involvement <br />In our nine years of experience of providing services for at-risk youth, we have found <br />that there is little parental involvement with youth at this age level and background. <br />Therefore, OCCC anticipates little to no involvement by the parents of the youth enrolled <br />in our proposed program. However, the OCCC WIA Coordinator will make every <br />attempt to involve parents in the program to determine participant needs, eligibility, <br />program success, and seek parent input into program design. OCCC will use a variety <br />of methods to gain parent involvement including phone calls, appointments, home visits, <br />surveys, recognition ceremonies and other activities that will include and promote <br />family/parent involvement. <br /> <br />Placement in Jobs or Continuing Education <br />As Corpsmembers near completion of OCCC participation, OCCC will assist with <br />employment search, referral and placement.
