EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period =—600_PERSONS (300 students/families attending the program on a <br />regular basis and 300 Open House families). <br />11. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Services to be provided: To provide disadvantaged families in Santa Ana with disabled children <br />the opportunity to participate in Creative Arts Program that prioritizes on therapeutic goals and <br />Early Intervention via parent and child music therapy classes, art education, music education and <br />community workshops for the entire family! <br />Program Goals: (1) To enhance disabled students' level of social integration, language skills <br />and self-esteem via our music therapy classes, community choir and art workshops that integrate <br />children and youth with and without disabilities, in a non -threatening, creative and therapeutic <br />environment; (2) To promote parent involvement and healthy family ties via our parent -child <br />(music therapy)classes and Open House Community Art Workshops for the entire family; (3) To <br />help prevent youth delinquency by diverting older at -risk students from negative behaviors and <br />influences to positive and productive creative behaviors, and most importantly, (4) To encourage <br />older At -risk Youth help assist and mentor younger disabled children during workshops. <br />Client characteristics: At -risk Youth and Children with Disabilities <br />How funds will be utilized: Most of the funds will be used to pay Program Staff: Open House <br />Community Art Workshops Program Coordinator, Art Teachers and Music Therapist.
