Number of unduplicated Santa Ana residents to be served with grant funds during the 12 - <br />month contract period = 300 PERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12 -month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />CASA de SALUD Family !Health clinic has been a leader in the treatment of diabetes since the <br />clinic was founded in 2000. Specifically, CASA provides outreach, education, screening, <br />intervention, and treatment services to a population at -high risk far developing diabetes and its <br />related complications. These services are provided to a population with little or no access to <br />comprehensive health care services. Funding this project, Providing Our Diabetics <br />Empowerment Resources (PODER), will ensure CASA's continued provision of these much <br />needed services. Diabetics and those at risk for the disease and its related complications are <br />educated about and empowered to manage their disease. Education will be provided by two <br />bilingual Promatoras (Community Health Outreach Worker) who have special training and <br />experience in teaching about diabetes.
