EXHIBIT A <br />Scope of Work <br /> <br />Annual Accomplishment Goal <br /> <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 1~700 PERSONS <br /> <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br /> <br />The focus of Project PRIDE will be to strengthen family unity for all family members by providing support and <br />family oriented activities in a structured and safe environment and by providing opportunities for increased <br />community involvement and volunteer service. Project PRIDE will consist of two major components, including the <br />Parent and Family Services and the Teen Services. Grant funds will be utilized to cover 50% of one full-time <br />supervisor and 100% of 24 pan-time staffproviding direct services. The goals and objectives of the program <br />include: <br /> <br />PARENT AND FAMILY SERVICES <br />Goal gl: To increase the level of interaction of families in drug-free environmena <br />· Nine Family PRIDE Clubs will meet once a month with 350 families and 1,500 members. <br />· A total of 200 fathers will participate in family and/or volunteer activities with a minimum of 125 fathers <br /> completing 10 family and/or volunteer activities each in a 12-month period. <br />· A total of 200 teens will participate in family, recreation and/or volunteer activities with a minimum of 75 <br /> completing 15 activities each in a 12-month period. <br />Goal #2: To increase parent and family involvement in the community. <br />· 50 parents will complete a two-day Leadership Development Training Retreat. <br />· A minimum of 200 parents will complete a total of 1,500 hours of volunteer service/community involvement <br />· 20 parent volunteers will serve on the Parent Commission and attend monthly planning meetings. <br />TEEN SERVICES <br />Goal #3: Academic enrichment activities will be developed and implemented in collaboration with SAUSD. <br />· 200 students at 4 intermediate schools will participate in academic enrichment activities after school. <br /> <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br /> <br />contract period per quarter: <br /> Quarter 1: July 1 - September 30 <br /> Quarter 2: October 1 - December 31 <br /> Quarter 3: January 1 - March 31 <br /> Quarter 4: April 1 - June 30 <br /> <br />Total <br /> <br />1~400 Persons <br />100 Persons <br />100 Persons <br />100 Persons <br />1,700 Persons <br /> <br />Invoicing Schedule <br /> <br />Estimated the mount of grant funds to be requested during the 12-month contract period on a <br /> <br />quarterly basis: <br /> Quarter 1: July 1 - September 30 <br /> Quarter 2: October 1 - December 31 <br /> Quarter 3: January 1 - March 31 <br /> Quarter 4: April 1 - June 30 <br /> Total <br /> <br />$ 79,762.5 <br />$ 79~762.5 <br /> 79,762.5 <br /> 79~762.5 <br /> 319~050 <br /> <br />Exhibit A <br /> <br /> <br />
