UN,._ ~;,,~d"~.~4- E^PIRES <br /> <br /> CLERK OF COUNCIL <br /> AMENDMENT TO AGREEMENT FOR THE rROVIS~ OF/ <br />INMATE MEDICAL SERVICES BETWEEN CITY OF SANTA ANA AND <br /> <br /> CO~CTIONAL MANAGED CA~ MEDICAL CO.OPTION <br /> <br /> This AMENDMENT TO AGREEMENT, made and entered into this ~o ~ day of <br />/((~ ,2000 by and between the City of Santa Ana, a charter city and municipal <br />corporation duly organized and existing under the constitution and laws of the State of California <br />("CITY") and CORRECTIONAL MANAGED CARE MEDICAL CORPORATION. <br />("CONTRACTOR") modifies the original contract and Exhibit A and D thereto entered into on <br />September 1, 1998. This AMENDMENT TO AGREEMENT is made in accordance to the <br />stipulations set forth in section I. ALTERATION OF TERMS, on page 2 of the original <br />agreement. <br /> <br />W-I-T-N-E-S-S-E-T-H <br /> <br /> The CITY wishes to establish a systematic and cost-effective program for the provision <br />and payment of off-site emergency medical care for Santa Ana Jail inmates. CONTRACTOR <br />has the ability to provide such services in addition to the on-site care that is currently in place. <br />The cost of such yearly off-site care should not exceed $80,000.00. Moreover, City wishes to <br />provide on-site emergency psychiatric crisis intervention and evaluation in addition to the current <br />on-site care. The costs of such additional services shall not exceed Ten Thousand Dollars <br />($10,000.00) per yearly period. In consideration of their mutual and respective promises, and <br />subject to the terms and conditions as set forth in the original contract dated September 1, 1998, <br />the parties hereto do hereby agree to modify the original contract and Exhibit A and D thereto as <br />follows: <br /> <br />A.
