c.) Contractor's address for Notices shall be changed as follows: <br /> <br />Correctional Managed Care Medical Corporation <br />1000 S. Anaheim Boulevard, Suite 203 <br />Anaheim, California 92805 <br /> <br />Attn: Rhoberta Paz, Director of Operations <br /> <br />d.) Contractor required Insurance Coverages shall be amended as follows: <br /> <br />Professional Liability <br />Comprehensive General Liability ) <br />Comprehensive Automobile Liability ) <br />Covering owned, non-owned and ) <br />Hired automobile hazards ) <br /> <br /> $3,000,000.00 <br /> $1,000,000.00 <br />Combined Single <br /> Limit <br /> <br />Paragraph D of Exhibit A, Section I, Payments, shall be amended to read as follows: <br /> <br />All payments for Chargeback Services identified in Exhibit D hereto shall not exceed <br />$105,500.00 per yearly period. This limitation of payments shall not obligate Contractor <br />to pay for services above this designated amount. <br /> <br />Payment for off-site emergency care and administrative costs shall be made sixty (60) <br />days after receipt of a detailed invoice statement of the inmate's name, date of service, <br />provider and amount billed. <br /> <br />3. Paragraph 6 of Exhibit A, Section III, Services, shall be amended to read as follows: <br /> <br />"6. Pharmaceuticals - Contractor shall provide routine over-the counter pharmaceutical <br />and routine prescription care for all inmates. Contractor will identify and maintain <br />appropriate levels of over-the-counter medications for inmate minor medical needs. <br />Long-term medication for chronic care, including HIV and psychotropic medications, <br />shall be the responsibility of the inmate's contracting agency." <br /> <br />Exhibit D - Cost Schedule for Chargeback Services, shall be replaced by the Cost <br />Schedule for Chargeback Services, attached hereto as Exhibit D and made a part hereof <br />by this reference. <br /> <br />A new Exhibit F- Staffing- shall be attached hereto and made a part hereof by this <br />reference. <br /> <br />Except as hereinabove modified, all terms and conditions of said Agreement and Amendment to <br />Agreement shall remain in full force and effect. <br /> <br /> <br />
