Mayne <br /> <br />Proposal for Consulting Services <br /> <br />This is to propose the renewal of my contract as an independent contractor with the City of Santa Ana for the period <br />January 1, 2004 -June 30, 2004. <br /> <br />The services provided will be of the same nature as detailed in my previous contract, namely services provided on <br />the Work Center site and offsite as part of the Rapid Response program. Services may also include editing of <br />workshop materials. Services will be provided on an as needed basis to be determined day-to-day by Work Center <br />staff and myself. <br /> <br />I propose to submit invoices to the City of Santa Ana for services rendered under the following terms. My hourly <br />rate will be $40, subject to change. Between January 1, 2004 and June 30, 2004, there are a possible 50 hours <br />during which I may provide W/O/R/K Center and Rapid Response assistance and materials editing, totaling no more <br />than $2,000. Invoices are payable by check and payment is due upon receipt. These terms are fixed. <br /> <br />Thank you for your consideration.
