<br />, <br /> <br />Exhibit H <br /> <br />Provider agrees that in addition to those agreements and obligations specified <br />in the contract boilerplate and scope of work, it will also adhere to and obey <br />the following provisions, assurances, and certifications. <br /> <br />ARTICLES <br /> <br />1) Provider agrees to serve target population specified in the Work Plan. <br /> <br />2) Provider agrees to recruit youth with ample time to deliver full program <br />objectives as specified in the Work Plan. <br /> <br />3) Provider agrees to meet or exceed Performance Benchmark Goal <br />Projections specified in the Work Plan. <br /> <br />4) Provider agrees to invoice Santa Ana WIB for all program <br />services/activities on a monthly basis. Provider will attach all supportive <br />documentation. <br /> <br />5) Provider agrees to provide participant the following W1A elements: <br /> <br />a. Work Experience / Internships (Paid & Unpaid) <br />b. Leadership Development <br /> <br />6) Provider agrees to provide participant access to all 10 WIA elements <br />through participation in the Youth Service Provider Network. <br /> <br />7) Provider assures debarment action has not been taken against named <br />organization. <br /> <br />8) Provider assures and certifies that it will not use any funds appropriated <br />under this agreement for religious activity or anti-religious activity, or to <br />promote or oppose any political candidate, parties, and/or beliefs. <br /> <br />9) Provider agrees to comply with the Americans with Disabilities Act by <br />providing equal access to individuals with disabilities in the public and <br />private sector services, transportation, communications and <br />employment. <br /> <br />10) Provider assures and certifies that any requested modification to this <br />agreement must be submitted in writing, explaining requested changes <br />and rationale. Provider further assures that no modification will be <br />implemented without prior written approval from the Santa Ana WIB <br />Youth Council. <br />
