11.1 <br /> <br />11.2 <br /> <br />11.3 <br /> <br />ARTICLE XI <br /> <br />11.0 EMPLOYEE INSURANCE <br /> <br />Health Insurance. The City shall contribute the following amounts toward the payment of <br />premiums for affected employees and their dependents under the California Public <br />Employees' Retirement System (CalPERS) health insurance programs. <br /> <br />Effective January 1, 2004, the City shall contribute toward medical premiums an <br />amount consistent with the rates then in effect for the "employee-only" and "family" <br />tiers respectively, of the Kaiser California CalPERS HMO plan. The "employee <br />only" tier applies to employees who have no dependents. The "family" tier applies <br />to employees who do have dependents. <br /> <br />Any contribution necessary to maintain benefits under any health insurance program <br />provided by the City for its employees and their eligible dependents in excess of the <br />amounts of the City contribution specified above shall be borne entirely by the <br />employee. <br /> <br />For each such employee who is covered under a spouse's non-City sponsored health <br />plan, the City will pay the employee a cash paymeut each month in an amount equal <br />to fifty percent (50%) of the monthly premium amount for the City's lowest <br />"employee-only" coverage, if said employee waives, in writing, City-paid coverage. <br />Said waiver shall include a provision warning such employee that reentry into any of <br />City-sponsored plans may require proof of insurability for such employee and/or <br />family. <br /> <br />Dental Insurance. The City agrees to contribute toward the payment of premiums for dental <br />insurance plans provided by the City for employees covered by this Agreement and their <br />eligible dependents on the following basis: <br /> <br />Effective January 1, 2004: <br /> <br />A. One hundred percent (100%) of the premium cost for "employee-only" coverage. <br /> <br />B. Up to seventy dollars ($70) per month per employee for "family" coverage. <br /> <br />Any contribution necessary to maintain benefits under said dental plans in excess of the <br />amount set forth above shall be borne entirely by the employee. <br /> <br />Disability Insurance. The City shall continue to pay one hundred percent (100%) of the <br />premium cost for a long term disability insurance plan for employees covered by this <br />Agreement as said plan was amended effective November 1, 1985, to provide Unrepresented <br /> <br />28 <br /> <br /> <br />
