f. Supportive services (linkages to community services, <br />transportation, childcare, housing, medical referrals, <br />uniforms or other appropriate work attire and /or tools; <br />g. Adult mentoring for a duration of at least 12 months, <br />(may occur both during and after program participation); <br />In. Follow -up services for not less than 12 months after the <br />completion of participation; <br />i. Comprehensive guidance and counseling, which may <br />include pregnancy prevention, drug and alcohol abuse, <br />counseling and referrals, as appropriate. <br />4. Provider shall establish at least one goal for each participant as <br />related to the elements identified in their ISS. If the participant <br />is deficient in basic reading and /or math skills, one primary goal <br />must be for basic skills and must be the first service provided. <br />Goals are to be based on appropriate age standards and entered <br />in participant's ISS. <br />5. Provider shall maintain and share with other Youth partners the <br />Individual Service Strategy (ISS) for each participant from time <br />of enrollment through and including twelve (12) months after <br />he /she is exited from the program. <br />6. Provider shall review program objectives with participant using <br />one -to -one method. At a minimum. review must include the <br />following information: <br />a. Program goals and objectives <br />b. Length of program period <br />C. Complaint handling procedures <br />d. Available supportive services <br />e. Anticipated exit date <br />f. Available services at the Santa Ana WORK Center <br />g. Follow -up phone calls & documentation requests <br />7. Provider shall provide participant services in accordance with <br />the program schedule and curriculum attached hereto as the <br />program proposal and Work Plan (Exhibit A & B). Neither the <br />program schedule nor curriculum may be altered in any way <br />during the term of this agreement, without prior written <br />approval given to Provider by the Santa Ana WIB Youth <br />Council. <br />
