EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Consultant shall perform the following services for City personnel, as requested by City: <br /> <br />Post exposure TB screen (including any necessary X-rays) <br />Post exposure blood screen (including HIV, Hepatitis B & C) <br />Blood lead level test <br />Asbestos screening <br /> <br />Annual flu vaccines for City employees <br /> <br />Annual Wellness physicals for City Fire Department personnel. <br /> <br />COMPENSATION <br /> <br />Consultant shall be compensated for each test administered as set forth in the schedule of fees <br />attached hereto. <br />Consultant shall be compensated in the amount of $10.00 per flu vaccine administered to City <br />employee. <br />Additionally, Consultant shall be reimbursed in the amount of $6150.00 for flu vaccines <br />administered to City employees during October 2002. <br />Total compensation to be provided pursuant to this Agreement shall not exceed $14,000.00 <br />during the term of this Agreement. <br /> <br /> <br />
