<br />D. <br /> <br />E. <br /> <br />the limitations of <br />subparagraph 3 above; <br /> <br />Section <br /> <br />A, <br /> <br />c. <br /> <br />The name, department, and <br />number of the recipient; <br /> <br />employee <br /> <br />d. <br /> <br />A statement indicating that the donor <br />understands such donation of time is <br />irrevocable. <br /> <br />3. <br /> <br />At the close of the two-week donation <br />period, the department shall verify that <br />each donating employee has accrued vacation <br />and/or comp time balances sufficient to <br />cover the designated donation. <br /> <br />4. <br /> <br />The department shall submit all approved <br />donation authorizations for an eligible <br />employee at one time for processing. No <br />donation authorizations for the eligible <br />employee will be processed after this <br />period. However, employees who receive <br />donations under this procedure and who <br />exhaust all donated hours may request an <br />additional donation period subject to the <br />provisions of Section A, herein. <br /> <br />5. <br /> <br />The City shall add the donated time to the <br />recipient's sick leave account. <br /> <br />Upon the death of any active employee receiving a <br />benefit under this provision, the City agrees to <br />pay any unused Catastrophic Leave Donations <br />remaining on the books to the survi ving <br />beneficiary. <br /> <br />For those current employees with unused <br />catastrophic leave on the books, the City agrees <br />to convert said unused catastrophic leave to sick <br />leave for use in accordance with the provisions of <br />Article XI, Sections 11.lA through 11.lK above. <br /> <br />52 <br />
