<br />Statement of Work <br /> <br />Radio Repair <br /> <br />1.0 Description <br /> <br />@ MOTOROLA <br /> <br />Radio Repair provides the component or board level service fur eligible portable and mobile <br />radio units ("Unit") listed in the then current CGISS Support Services Price Schedule. <br />Services are perfurmed at the Motorola Radio Support Center (RSC). <br /> <br />Motorola may use remanufactured or used parts. Radio Repair includes accessory items such <br />as but not limited to, mobile microphones and power supplies, only where they are required <br />fur normal operatiDn of the Equipment and are included at the point of manufacture. <br />Motorola reserves the right to exclude certain models of Equipment from coverage under this <br />Statement of Work and to exclude repairs to optional accessories, mobile antennas, cables, <br />and reprogramming. Services under this Statement of Work includes only those products <br />listed in the Radio Repair product pages in the USCSD Support Services Price Schedule. <br />This service does not cover Equipment defects, malfunctions, performance failures, or <br />damages to the Unit as listed in Section 4 of the Service Agreement. Radio Repair is non- <br />cancelable and non-refundable for the first twelve months after the Start Date or for the first <br />twelve months after Equipment is added to the Service Agreement. The terms and <br />conditions ofthis Statement of Work takes precedence over and is attached to the Motorola <br />Service Agreement. <br /> <br />2.0 Motorola has the fullowing responsibilities: <br />2.1 Test and return the Equipment to Motorola factory specifications. <br />2.2 Reprogram Equipment necessary to return Equipment to normal operations, based <br />on the template provided by the Cnstomer or a generic template fur that Equipment <br />if no template is provided, <br />2.3 Motorola will pay the freight charges for next day shipping to and from the Radio <br />Support Center provided that Customer uses the UPS Consignee Billing Program. <br /> <br />3.0 Customer has the following Responsibilities: <br /> <br />Radio Repair <br /> <br />3,1 <br />3.2 <br /> <br />Sign up for the UPS Consignee Billing Program. <br />Complete a Repair Request Form and submit it with each Unit sent in fur service. <br />Alternatively, complete and submit an electronic version of this form available on <br />Motorola's web site for each Unit. <br />Send Equipment along with a completed Repair Request Form to the Radio Support <br />Center for Service, Accessory items sent in without the Unit must reference the <br />serial number of the Unit on the repair request form. <br />Supply Motorola with Software templates fur Equipment needing to be <br />reprogrammed according to Customer specifications. <br />Customer must use the UPS Consignee Billing Program for inbound shipments to <br />the Radio Support Center. Motorola will not pay inbound shipping charges fur <br />shipments by other methods or carriers. <br />Pay freight insurance charges for declared values in excess of $1 00 on inbound <br />shipments. <br /> <br />3.3 <br /> <br />3.4 <br /> <br />3.5 <br /> <br />3.6 <br /> <br />Page I ofl <br />
