EXHIBIT A Scope of Work Annual Accomplishment Goal 1. Number of un duplicated Santa Ana residents to be served with grant funds during the 12- month contract period = 112 PERSONS II. Description of Work - in space below, describe the program to be funded during the 12-month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. The goal of the Southwest Senior center is to promote the availability of a fun range of senior programs, services and activities for the residents of the City, with particular attention given to address the needs of seniors whose interests are unique in relation to income, education, ethnicity and ftailty. The programs and services will include: . Conduct two senior related seminars to 84 adults over 55 years old . Facilitate a Senior Advisory committee comprised of 12 senior volunteers . Provide afternoon programs such as dance, art and health classes to 28 seniors . Conduct one senior health fair city-wide The funding will also be used to conduct city-wide senior outreach programs to encourage adults 55+ to actively get involved with community programs.
