EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goei <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 3-month <br />contract period = 1110' PERSONS <br />11. Description of Work - in space below, describe the program to be funded during the 3-month contract period <br />including, setviooa to be provided, program goals, client characteristics, & how grant funds wdl be utilized. <br />• Assistance in Meeting Educational Requimments: The Foundation will supply Drug <br />Court Participants with the information on educational opportunities and link participants <br />with Community College interns to assist with the admissions process and financial <br />opportunities. <br />• Supplement Program Assistance: The Foundation will complement the efforts of the <br />Drug Court team by linking participants with existing publicly and privately funded <br />agencies that identify housing opportunities, address medical/dental needs, provide job <br />ttaming/employment information and offer assistance with transportation. <br />• Alumni Continuing Care Program: The Foundation will establish a Drug Court <br />Alumni Continuing Care Program that will include graduates as well as those is the Drug <br />Court Program. This program will provide a support group for graduates and participants <br />by offering opportunities to remain in contact with the Drug Court Program by mentoring <br />new Drug Court participants, participating in panels or prevention programs and <br />organizing activities to keep graduates involved in a sober and healthy lifestyle <br />• Opportunities to Satisfy the Cultural and Civic Required Activities: The Foundation <br />will arrange opportunities for the participants to experience Cultural and Civic events that <br />encourage healthy interaction with other participants and family.
