<br />REQUEST FOR <br />COUNCIL ACTION <br /> <br />~ <br />~ <br /> <br />.ITY COUNCIL MEETING DATE: <br /> <br />CLERK OF COUNCIL USE ONLY: <br /> <br />AUGUST 2, 2004 <br />TITLE: <br /> <br />APPROVED <br /> <br />,r ,". <br /> <br />tlAz, <br /> <br />CITY MANAGER <br /> <br />0 As Recommended <br />0 As Amended <br />0 Ordinance on 1st Reading <br />0 Ordinance on 2nd Reading <br />0 Implementing Resolution <br />0 Set Public Hearing For <br /> <br />APPROVE AN AGREEMENT FOR <br />HAZARDOUS MATERIALS RESPONSE <br />TEAM MEDICAL EXAMINATIONS <br /> <br />CONTINUED TO <br /> <br />FILE NUMBER <br /> <br />RECOMMENDED ACTION <br /> <br />Direct the City Attorney to prepare and authorize the City Manager and the <br />Clerk of the Council to execute an agreement with Pulmonary Consultants & <br />Primary Care Physicians Medical Group Inc. to perform annual Hazardous <br />8aterial medical examinations in an amount not to exceed $20,000 per year <br />or a three-year agreement beginning July 1, 2004 with one, two-year <br />renewal option. <br /> <br />DISCUSSION <br /> <br />Under Title 8, Section 5144 of the CAL-OSHA Reparatory Protection <br />Regulation, members of the Fire Department's Hazardous Materials Response <br />Team are required to undergo an examination to determine fitness for duty. <br />The physical examination includes, but is not limited to, a detailed <br />medical history questionnaire, screening audiometry, chest X-ray, pulmonary <br />function test, and cancer screening. <br /> <br />FISCAL IMPACT <br /> <br />Funds are available in FY 2004/2005 in the Fire Department's Fire <br />Suppression Other Contractual Services account (account no. 11-323-6291). <br /> <br />APPROVED AS TO FUNDS AND ACCOUNTS: <br /> <br />tV~ t/~ <br />~.CPhilliP M.
