JUN -1-x004 01:53 FROM:K RUTLAND / CASA DE 7145713956 TD:6476c�49 <br />Community Development Block Grant <br />Revised Scope of Work Prepared for the City of Santa Ana <br />BUDGET NARRATIVE <br />Personnel: <br />Two Promatores (Community Health Outreach Worker) Budgeted @ $38,133 (2 FTE' <br />NOT REQUESTED in this grant. Will provide direct education and prevenour <br />information to diabetes patients and their families either on a group basis in communh <br />centers, churches, and at work sites, or on a one-to-one basis as required. Promotora will <br />be trained by CASA's consultant Health Educator to provide classes on health subjects <br />related to diabetes and its disease related complications. Based on limitations and budget <br />constraints, this cost will be covered by other funding sources. <br />Fringe Benefits (22%).NOT REQUESTED IN THIS GRANT. In order to remair <br />competitive in the current marketplace and retain competent employees, CASA's fringe <br />benefit rate is 22%. This rate consists of the following components; FICA (6.2%) Medt- <br />Care (1.5%), FUTA (0.8%), SUI (3.4%), Workers Compensation (4.0%), Health <br />Insurance (6.01/6). Based on a 22% fringe benefit rate, a total of $8,389 is allocated in the <br />project budget. Based on limitations and budget constraints, this cost will be covereo b, <br />other funding sources <br />Operational Costs: <br />Other• <br />Educational Materials: The amount needed for these materials is calculated at $6.00 per <br />contact made (1963) a total of $11,780 is allocated in this project budget. <br />Indirect Cost: NOT REQUESTED IN THIS GRANT. Calculated at a rate of 150/,o of <br />subtotal personnel, this totals $5,720 for cost of management and administration, human <br />resources, accounting purchasing, information systems for the clinic. Based on limitation, <br />and budget constraints, this cost will be covered by other funding sources. <br />Consultants: NOT REQUESTED IN THIS GRANT. Budgeted @ $12,040 the Health <br />Education Consultant will identify community multicultural health needs, develop and <br />conduct health education training classes/interventions for health care professionai <br />develop modules for the Community Health Outreach Workers (Promotoras) that would <br />include training, develop multilingual/multicultural education materials/promotion <br />establish and implement fielding -testing criteria; collect and interpret data; implement <br />community assessment strategies; and supervise the Community Health Outreach Worker, <br />and assure/ensure the assigned scope of work is performed. Based on limitations and <br />budget constraints, this cost will be covered by other funding sources. <br />
