Description of Work — in space below, describe the program to be funded during the 1? mon <br />contract period including, services to be provided, program goals, client characteristics. & he <br />grant funds will be utilized. <br />The primary need that we will be fulfilling for the elderly residents of Santa Ana in om scn, <br />program is to provide services, access to information, and the critical socialization than th, <br />elderly need in order to keep them active in society. <br />Goal 1: Link Seniors to health and human services. <br />Goal 2: Improve Senior health and well-being by providing a forum for socialization <br />Goal 3: Increase Seniors access to critical information and resources <br />Nearly all beneficiaries of our program —especially social service programs — should be u! <br />"low/moderate-income. " Under this criterinn, the program must provide benefits m Santo I <br />residents whose household incomes are not more than 80% of Orange Counh 's median <br />income, adjusted for family size.
