Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />The goal of Human Options is to help battered women, their families and our community break <br />the cycle of domestic violence. In order to do this we assist women who are homeless because <br />of domestic violence and move them toward self-sufficiency, emotionally, economically, and <br />socially. <br />50-60 homeless Santa Ana residents a year would meet 6-8 hours weekly during a 45 day stay <br />with the Counselor III being funded by this grant. The daily sessions would assist each client in <br />these ways: <br />• Assistance in obtaining permanent housing <br />• Medical and psychological counseling <br />• Employment counseling <br />• Nutritional counseling <br />• Assistance in obtaining other Federal, State, and local assistance including mental <br />health benefits; employment counseling; medical assistance; and income support <br />assistance such as SSI benefits, AFDC, GA, Food Stamps <br />• Childcare services and assistance, transportation, job placement and training <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: <br />July 1 — September 30 <br />Quarter 2: <br />October 1 — December 31 <br />Quarter 3: <br />January 1 — March 31 <br />Quarter 4: <br />April 1 — June 30 <br />Invoicing Schedule <br />14 <br />Persons <br />14 <br />Persons <br />13 <br />Persons <br />14 <br />Persons <br />Total 55 <br />Persons <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: <br />July 1 — September 30 <br />$ <br />5,000 <br />Quarter 2: <br />October 1 — December 31 $ <br />5,000 <br />Quarter 3: <br />January 1 — March 31 <br />$ <br />5,000 <br />Quarter 4: <br />April 1 — June 30 <br />$ <br />5,000 <br />Total $ <br />20,000 <br />EXHIBIT A <br />
