CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12-month <br />contract period= 50 PERSONS 450 total = 11% Santa Ana clients <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be <br />utilized. <br />Goal: To help low-income people access quality health services and navigate the delivery systems so that they can <br />obtain critical medical services for which they are entitled to as well as empowering consumers to be active <br />participants in their own healthcare and in planning programs designed to serve them. Although the HCAC provides <br />outreach services in addition to direct client services, LASOC is requesting funding to assist with direct services. <br />The following objectives relate to the direct service component of HCAC. <br />Obiective(s): <br />1. Help 50 low-income residents of Santa Ana resolve problems related to accessing critical medical services, <br />which if not obtained, could increase their risk of serious illness or even death. Act as legal advocates on behalf of <br />clients to ensure that residents receive much -needed medical care regardless of income. <br />Task/Action: <br />(A) Screen applicant for eligibility through the HCAC Hotline Monday through Friday and provide advice and <br />counsel to eligible clients. <br />(B) Represent clients who need a legal advocate to intervene. <br />2.
