1. Describe the methodology to be followed for each service identified in the "Scope of Work"; Offerors shall explain the step-by-step procedures to be followed when performing the service. 2. Include a schedule showing the time required for completion of the project. The schedule should reflect the major project specific measurements and milestones and the estimated time it will take to perform each service required. D. Company Data 1. State the company's legal name and address and the names and titles of its principal officers. Indicate type of entity and indicate if your finn is incorporated and, if so, furnish the state of incorporation; 2. If your company is a sole proprietorship doing business under a different name, indicate the sole proprietor's name and the name(s) you are doing or have done business as (DBA) or also known as (AKA); 3. Provide your company's Federal Employer I.D. Number; 4. State the name and address of the person to receive notices and who is authorized to make decisions and répresent the company and specify in what capacity the person shall be representing the company and any limitations on their authority. 5. Indicate the company's number of years in business. E. Orqanization Chart/Staffin!! Plan 1. Provide an organizational chart that reflects the title of key staff and the management contracts of each individual assigned to provide the services under the proposed Agreement; and 2. Provide resumes of all personnel who will perfonn work under this Agreement. Indicate the number of years they have had in preparing 5 Year Consolidated Plans, Annual Action Plans and Citizen Participation Plans. Include any proposed subcontractor(s) and their staff. 3. Provide job description for the classification of each employee, the names and titles of the key personnel assigned to the project and the amount of time they will dedicate to the project. 4. List the names, titles, experience and job duties of all subcontractors and the amount of time they will dedicate to the project. F. References Provide a minimum of three references for which Offeror has prepared 5-Year Consolidated Plans, Annual Action Plans or Citizen Participation Plans. Include the name of the organization, jurisdiction, City or County, name and telephone number of contact person, description of services provided, date and amount of contract. Provide three sample work products which include 5-Year Consolidated Plans, Annual Action Plans and/or Citizen Participation Plans. The sample products need only be included in one of the four proposals. G. Resources to be provided by the City 1. The City will provide all copies of survey and letters to be mailed to all interested parties. 2. The City will provide all envelopes, mailing labels and postage for the distribution of Needs Surveys developed by the Consultant to be mailed to all interested parties. 3. The City will provide translation of written materials as necessary
