CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />Total number of clients anticipated to be served during the 12-month contract period= <br />2,100 PERSONS <br />lI. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12-month contract period= 240 PERSONS 11% Santa Ana clients <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics. & how grant funds will be utilized. <br />The Transitional Program, which lasts from 2 - 6 months, provides up to 14 families with <br />a private bedroom and communal kitchen and living room area (maximum of four <br />families use each communal space). Families eligible for this program must agree to <br />save 80% of their income, find employment, set weekly and long-term goals and <br />participate in classes and counseling sessions. Family Advisors work closely with the <br />whole family to lend emotional support and build stability. The Transitional Program <br />fosters the development of personal and fiscal responsibility in order to encourage <br />residents to become self sufficient and contributing members of society. Each year <br />more than 20 families "graduate" and achieve a measure of independence. Orange <br />Coast Interfaith Shelter now has a Second Step Transitional Housing Program (an <br />additional 12-18 months in length) to complete the continuum of supportive shelter for <br />homeless families. When families are successful in the very structured Transitional <br />Program, this allow them to gradually lessen the amount of support provided by OCIS <br />over a period of time, while continuing to work on family issues. <br />Schedule ofPerfarmance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July I - September 30 60 Persons <br />Quarter 2: October 1 — December 31 60 _ Persons <br />Quarter 3: January l — March 31 _ 60 _ Persons <br />Quarter 4: April l — June 30 _ 60_ _ Persons <br />Total _ 240 Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July I — September 30 $_ 1,650 _ <br />Quarter 2: October t — December 31 $ 1,650 <br />Quarter 3: January I — March 31 $_ 1,700 _ <br />Quarter 4: April I — June 30 $ <br />Total $ 5,000 <br />EXHIBIT A <br />
