CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to be served during the 12-month contract period= 400 <br />PERSONS <br />II. Total number of unduplicated Santa Ana residents to be served with grant funds during the 12-month <br />contract period = 160 PERSONS (80 YOUTH/80 PARENTS) 40% Santa AAia CDBG cl ient <br />III. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be <br />utilized. <br />Programa Shortstop provides services to youth offenders, ages 10-17, and their families. First time offenders are given the <br />chance to avoid prosecution by participating in a rigorous program of learning about the consequences of their offenses in <br />REAL courtrooms with legal professional facilitators. Services provided include 1) Intervention services through two <br />SHORTSTOP program sessions-8hours; 2) Case Management, Counseling, and Referral Services -as needed. <br />Our program goals include 1) To serve 80 youth and 80 family members in Programa Shortstop, with 89% successful <br />completion, 2) Provide Santa Ana families with additional referrals upon completion of Programa Shortstop, as needed, 3) <br />Disseminate 240 copies of the Laws for Youth booklet in Spanish to Santa Ana residents, and 4) Disseminate 240 copies <br />of the OCBF Substance Abuse Awareness booklet in Spanish to Santa Ana residents. <br />The characteristics of our clients include: high -risk population living in economically strained neighborhoods of Santa <br />Ana; are a minority (Latino); and they have been identified by a law enforcement agency, a school, a parent/family <br />member or a community agency as participating in delinquent or illegal behavior. They also face additional risk factors <br />because of cultural biases and exposure to crime and/or violence. They are at extremely high risk of substance abuse, <br />delinquency, gang involvement, risky sexual behavior, teen pregnancy, dropping out of school, and being victimized (due <br />to their social, family, and community environments, as well as their low economic status). <br />The funds received from CDBG will go toward the salary of our Programa Shortstop Coordinator who is responsible for <br />the coordination of Programa Shortstop sessions, which covers referrals through program completion and reporting. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month contract period per <br />quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 - June 30 <br />Invoicing Schedule <br />40 <br />Persons <br />40 <br />Persons <br />40 <br />Persons <br />40 Persons <br />Total 160 Persons <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a quarterly basis: <br />Quarter 1: July 1 — September 30 $ 3,500 <br />Quarter 2: October 1 —December 31 $ 3,500 <br />Quarter 3: January 1 — March 31 $ 3,500 <br />Quarter 4: April 1 — June 30 $ 3,500 <br />Total $ 14,000 <br />EXHIBIT A <br />
