Description of Work - in space below, describe the program to be funded during the 12- <br />______.,____.month contract period including, services to be provided, program goals, client <br />At -risk youth (students identified with behavioral, academic, emotional problems) will be given the opportunity <br />to learn new skills (via arts & computer inst), divert negative behaviors or influences to productive ones, and <br />enhance educational opportunities. This program will teach them to learn about differences, disability <br />awareness, and about the process of peer tutoring and to collaborate with students that have a disability. This <br />experience will teach at -risk youth to value life and the opportunities that they have been given to make things <br />better for themselves and their community. This experience will have a positive impact on students level of <br />empathy and moral responsibility; foster their self-esteem, and emotional well being. The objectives of the <br />program are: to provide an integrated arts and technology program 2 times a week to approximately 24 children <br />with disabilities and 12 mentors of limited resources; to teach at -risk youth to collaborate and tutor disabled <br />children/youth during computer instruction time, and help foster a spirit of collaboration, peer tutoring and <br />disability awareness; to enhance the educational opportunities, creative/artistic expression and potential of at - <br />risk and special needs children and youth; to foster self-esteem and overall emotional well-being of students <br />through their artistic accomplishments and educational experiences Specific outcomes include the following: 80 <br />to 90% of students will develop or increase their artistic and computer knowledge and skills as measured by pre <br />and post-tests, and portfolios of students and will benefit in at least one of five basic areas (social/emotional, <br />motor, language/communication, cognitive, academic) as determined by students educational/therapeutic plans. <br />Schedule of Performance <br />Estimate the number of -unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July I — September 30 9 Persons <br />Quarter 2: October i - December 31 9 Persons <br />Quarter 3: January 1 — March 31 (4 Persons <br />Quarter 4: April 1 — June 30 9 Persons <br />Total 3 (P Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ 21 $ D $ . <br />Quarter 2: October 1 — December 31 $ 2 , fn D $ <br />Quarter 3: January I — March 31 $ �' (� n- <br />Quarter 4: April 1 — June 30 $— j 9 -77 5 <br />Total $ 1 0 ) <br />EXHIBIT A <br />
