CITY OF SANA ANA COMMUNITY DEVELOPMENT BLOCK GRANT Scope of Work Annual Accomplishment Goal I. Total number of clients anticipated to be served during the 12-month contract period= 17+ PERSONS I1. Total number of unduplicated Santa Ana residents to be served with grant funds during the 12-month contract period = 17 PERSONS (households) III. Description of Work - in space below, describe the program to be funded during the 12- month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. The descriptive goals remain the same as identified in original grant proposal: 1) Identify low-income and/or disabled homeowners in need of assistance to make their homes safe, accessible, secure, warm and dry. 2) Provide the resources to rehabilitate an increased number of qualified owner -occupied homes in the City of Santa Ana. 3) Continue development of our capability to accomplish year-round home modifications for homeowners with disabilities. With lower.level of funding, and as our work is "incremental", we will still be able to accomplish the above, however to a lesser degree.
