CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to be served during the 12-month contract period= <br />17+ PERSONS <br />I1. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12-month contract period = 17 PERSONS (households) <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The descriptive goals remain the same as identified in original grant <br />proposal: <br />1) Identify low-income and/or disabled homeowners in need of <br />assistance to make their homes safe, accessible, secure, warm <br />and dry. <br />2) Provide the resources to rehabilitate an increased number of <br />qualified owner -occupied homes in the City of Santa Ana. <br />3) Continue development of our capability to accomplish year-round <br />home modifications for homeowners with disabilities. <br />With lower.level of funding, and as our work is "incremental", we <br />will still be able to accomplish the above, however to a lesser degree. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 —December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />2 Persons <br />4 Persons <br />S Persons <br />Persons <br />Total 17 Persons <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ i , 800 <br />Quarter 2: October 1 — December 31 $ 3,500 <br />Quarter 3: January 1 — March 31 $ 4 , 4 on <br />Quarter 4: April 1 — June 30 $ S 3110 <br />Total $1T000 <br />EXHIBIT A <br />
