CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to be served during the 12-month contract period= <br />3�,975� PERSONS <br />U. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12-month contract period = 2,700 PERSONS <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Saint Joseph Ballet provides comprehensive, integrated programs --dance training, performance <br />opportunities, academic services, college scholarships, family services, mentoring and enrichment, and <br />community outreach —that support the educational, social, and personal growth of low-income youth and <br />families. <br />Our mission is to empower low-income youth from diverse backgrounds to strengthen self-esteem, self- <br />discipline and a sense of accomplishment through dance, academic, and family programs. We intend to <br />serve at least 288 Santa Ana youth and 200 parents with our Year -Round Programs who qualify under the <br />"low/moderate" income criterion, and 2,212 low/moderate income Santa Ana students through our <br />outreach program, DanceFree Weeks. Further, Saint Joseph Ballet has a goal to strengthen and deepen <br />the impact of our services on youth and families. Objectives include college assistance for high school <br />students, academic tutoring for at least 190 Santa Ana students, free family education seminars, and <br />assistance for Santa Ana families in need of crisis intervention and/or referrals to other community <br />resources. <br />A typical student in our Year -Round Programs is from a family of five living on an average income of <br />$2,759/month; 90% qualify as low-income and participate free of charge. Funds will be utilized to cover <br />a portion of the salaries of program staff positions: Student Coordinator and Academic Coordinator. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />256 Persons <br />2,434 Persons <br />Persons <br />Persons <br />Total 2,700 Persons <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ 15,185 <br />Quarter 2: October 1 — December 31 $ 15,410 <br />Quarter 3: January 1— March 31 $ 12,405 <br />Quarter 4: April 1 — June 30 $ <br />Total $ 43,000 <br />EXHIBIT A <br />
