CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to be served during the 12-month contract period= <br />7,500 PERSONS <br />II. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12-month contract period = 1,700 PERSONS 23% Santa Ana CDBG clients <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The SOS Free Medical and Dental Clinic will provide comprehensive, free medical and dental <br />services to 1,700 Santa Ana residents. The program goal is to protect the health of the <br />community by providing medical and dental services to those with no other access to that <br />service. Medical services include diagnosis, treatment, education, lab work, pharmacy and <br />comprehensive access to specialists. Dental services include hygiene, partials, root canals, <br />radiographs, restorations, extractions and dentures. The patients are low-income Santa Ana <br />residents who do not qualify for government insurance programs. Patients are primarily <br />Hispanic.
