EXHIBIT A <br />SCOPE OF SERVICES <br />Consultant shall assist individuals who are returning from incarceration to overcome the <br />obstacles that may prevent them from transitioning into the community. <br />During the term of this Agreement Consultant, through the St. Vincent Center, will provide the <br />following services: <br />• Intake and assessment of at least 100 individuals who are re-entering the community <br />from the Santa Ana Jail, County facilities or California Corrections. <br />• Track and provide reports on the progress of the individuals referred to the program. <br />• Provide job assessment, resume writing, interviewing skills training and job retention <br />assistance. <br />• Provide or refer individuals to family counseling, substance abuse groups/counseling, <br />anger -management and tattoo removal. <br />• Identify issues or obstacles that prevent individuals from changing their lives and seek <br />means to overcome those obstacles. <br />Methodology <br />Consultant will use various methodologies to assist individuals in successfully re-entering the <br />community. Those methods include: <br />• Assessment and follow-up with individual case management <br />• Referral and information services regarding job training and placement, housing and <br />family reconciliation <br />• Group support and individual counseling for families <br />• Provide skills classes on filling out applications, how to dress and interviewing <br />techniques <br />• Provide mediation sessions with family members <br />Evaluation <br />Consultant will track cases and provide evaluation of the success/failure of the program at the <br />conclusion of the term. Consultant will provide a monthly report to the Santa Ana Police <br />Department, including services rendered, cases completed and tracking data. Said reports shall <br />include: <br />• Pre -assessment and post assessment interviews with the clients <br />• Attendance sheets/counseling attendance records <br />• Follow up surveys <br />• Six-month review for re -contact with SAPD <br />
