EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period: 532 PERSONS f„ ; <br />II. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Our client base for this grant will consist of low-income families, senior citizens, those who are <br />newly homeless and those who are transitioning from homeless shelters into more permanent <br />housing. Our goal is to assist these clients with direct services and referrals, as well as life skills, <br />to become self-sustaining. <br />Specifically, we will provide those clients who meet the income and residency requirements with <br />funds to help pay a portion their rent to avoid eviction (eviction prevention) or defray a portion <br />of their move -in (rental assistance) costs. As is our usual practice, we will conduct an extensive <br />intake interview first via telephone and then face-to-face when they bring documentation of their <br />need. We will recruit and train volunteers to conduct both types of interviews to supplement our <br />already limited staff. To avoid duplication of service and misuse of funds, we will continue to <br />use the Community Action Partnership of Orange County's Clearinghouse to help make the <br />determination on each client's request. We will routinely conduct these interviews on Tuesdays, <br />when we have limited client services in other areas, to ensure client confidentiality. <br />Similarly, we will provide utility assistance to those families who are in jeopardy of having their <br />utilities (electric and gas) services terminated. We will use the same type of intake and <br />documentation process as we use for eviction prevention and rental assistance. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 40 Pare <br />Quarter 2: October I — December 31 170 Persons <br />Quarter 3: January 1 — March 31 112 Persons <br />Quarter 4: April 1 — June 30 210 persons <br />Total 532 Persons <br />Invoicing Schedule <br />Estimated the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ 3.000.00 <br />Quarter 2: October 1 —December 31 $ 7,500.00 <br />Quarter 3: January 1— March 31 $ 5,000 00 <br />Quarter 4: April 1 — June 30 $12.500 00 <br />Total $28000 00 <br />
