CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to be served during the 12 -month contract period= <br />125 PERSONS <br />II. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12 -month contract period= 30 PERSONS 24% Santa Ana CDBG clients <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />1. Thomas House Temporary Shelter is a transitional homeless family, with children shelter. <br />2. Services to be provided are: food, lodging, case management, job search, child care, tutoring, <br />mental health counseling, tutoring and other life skill services as needed. <br />3. The program has one major goal and that is to have every homeless family that enters the <br />shelter move to a permanent residence, independence and a self - sufficient life style. <br />4. All clients of the shelter are homeless families both one and two parent, with children. Thomas <br />House allows the family to remain an entity as it progresses to independence and self - <br />sufficiency. <br />5. The funds awarded as a result of this submission will be used for salaries, food and supplies. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12 -month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January I — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />8 Persons <br />8 Persons <br />7 Persons <br />7 Persons <br />Total 30 Persons <br />Estimate the amount of grant funds to be requested during the 12 -month contract period on a <br />quarterly basis: <br />Quarter 1: July i — September 30 $ 3,750 <br />Quarter 2: October 1 — December 31 $ 3,750 <br />Quarter 3: January 1— March 31 $ 3,750 <br />Quarter 4: April 1 — June 30 $ 3,750 <br />Total $ 15,000 <br />EXHIBIT A <br />
