EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />At least 80 victims of domestic violence and their children will receive shelter, food, clothing <br />and supportive services through the 45-day emergency shelter program. The adult counseling <br />program consists of five primary diagnoses (those most often seen in this population) and ten - <br />session increments for each of the diagnoses (Depression, Generalized Anxiety, PTSD, <br />Dependent Personality Disorder, and Substance Abuse — others as needed). Goals of this <br />program would be to provide a safe haven for victims of abuse/violence and their children, then <br />concentrate on healing the effects of the violence through an intense counseling program. The <br />counseling program focuses on education regarding the causes of violence in the home, the <br />effects of the violence and abuse and how to become independent from the dependence on a <br />batterer and/or substance abuse. Overall goals include: a decrease and cessation of substance <br />abuse, decrease in depression and anxiety, and a mental health stability through counseling and <br />any necessary medication. Grant funds will be utilized for operational costs such as <br />communications, insurance, maintenance/repair and utilities. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 20 Persons <br />Quarter 2: October 1 — December 31 15 Persons <br />Quarter 3: January 1 — March 31 25 Persons <br />Quarter 4: April 1 — June 30 20 Persons <br />Total 80 Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis <br />Quarter 1: July 1 — September 30 $ 6,000 <br />Quarter 2: October 1 — December 31 $ 6,000 <br />Quarter 3: January 1 — March 31 $ 6,000 <br />Quarter 4: April 1— June 30 $ 6,000 <br />Total $ 24,000 <br />
