<br />As a reminder, the City Clerk's Office will no longer accept documents without proper identification as .
<br />specified in the memo from the City Clerk dated November 1, 2006. Documents including insurance
<br />notifications, certificates, etc., must be accompanied by a contract, agreement, or agreement number,
<br />agency name, agency contact information, and approval from the City Attorney's Office.
<br />
<br />In addition to the request made by the City Clerk's Office, the City Attorney's Office will require this
<br />identifying information on requests for agreements, contracts,. certificates of insurance, and any other
<br />document requiring approval from the City Attorney. Currently, the City Attorney's Office is receiving
<br />documents with no identifying information (contract/agreement number, agency information; project
<br />manager). Without this information, the documents cannot be processed or delivered to the proper
<br />agency.
<br />
<br />In an effort to minimize the amount of "orphan" documents that the office receives, these documents
<br />will be held and destroyed after 30 days. Please forward this information to the appropriate personnel
<br />in your agency.
