2004 -2005 <br />CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of clients anticipated to serve during the 12 -month contract period= 20 <br />PERSONS <br />II. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12 -month contract period = 20 PERSONS 100% Santa Ana Clients <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The goal is to provide new and innovative ways to empower Santa Ana Latino <br />Communities and particularly to those in single parent household or those exposed to domestic <br />violence and child or substance abuse, to emotionally and physically cope with their varying <br />situations. Currently, many of these children and their custodial and non - custodial parents have <br />no idea how to lead normal emotionally healthy lives. This program is designed to give them <br />these survival tools and concentrate on the children in order to overcome the cycle of despair and <br />violence. <br />California Hispanic Commission on Alcohol & Drug Abuse, Inc. will assist in providing <br />the necessary survival tools to be able to function in families that are high risk of abuse — <br />physically, verbal, emotional, sexual, harmful to self or any family member. California Hispanic <br />Commission on Alcohol & Drug Abuse, Inc. provides a safe, educational environment with <br />experienced and highly trained personnel. <br />The curriculum is a 12 week session encompasses cultural issues, child growth and <br />development coping with abuse and neglect, communication skills, anger management, <br />discipline techniques, power and respect, parental identity, substance abuse issues and family <br />dynamics. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12 -mouth <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 5 Persons <br />Quarter 2: October 1 — December 31 5 Persons <br />Quarter 3: January 1 — March 31 5 Persons <br />Quarter 4: April 1 — June 30 5 Persons <br />Total 20 Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12 -month contract period on a <br />quarterly basis: <br />Quarter 1: <br />July 1 — September 30 <br />$ <br />5,000 <br />Quarter 2: <br />October 1 — December 31 <br />$ <br />5,000 <br />Quarter 3: <br />January 1 — March 31 <br />$ <br />2,500 <br />Quarter 4: <br />April 1 — June 30 <br />$ <br />2,275 <br />Total <br />$ <br />14,775 <br />EXHIBIT <br />A <br />
